Notice of Data Security Incident

What Happened?

On June 21, 2018, we learned that an unauthorized third party gained access to one of our secure email accounts. We immediately began an investigation and terminated any unauthorized access to the email account. The investigation determined that the unauthorized access occurred between June 7, 2018 and June 21, 2018. All of our email accounts, systems and information remain secure.

What Information Was Involved?

Our investigation confirmed that the following types of information related to our clients and employees may have been exposed as a result of the incident:

  • For clients – the information included name, an indication that the individual is or was a client, or potential client, of Authentic Recovery and limited clinical information might have been exposed in this incident. Of the affected clients, one individual also might have had his or her payment card information exposed in the incident.
  • For employees – the information included name and driver’s license number. Of the affected employees, two individuals also might have had their address, phone number, date of birth and social security number exposed in the incident.

What You Can Do?

Out of an abundance of caution, Authentic Recovery is offering credit monitoring and identity theft recovery services, at no cost, to potentially affected individuals. Potentially affected may consider this no-cost service.

Although there is no evidence that any of this information has been misused, as an added precaution, we recommend that, in the event potentially affected individuals notice any suspicious activity, they may wish to order copies of their credit reports from each of the three national credit reporting agencies to check for any inaccurate information, particularly medical services or medical bills that are not recognizable. Potentially affected individuals may obtain their free annual credit report from each of the national credit reporting agencies by visiting www.annualcreditreport.com, by calling 1-877-322-8228 or by mailing their request to Annual Credit Report Request Service, P.O. Box 105281, Atlanta, GA 30348-5281.

What We Are Doing?

Upon learning of this incident, Authentic Recovery immediately moved to confirm whether personal information may have been exposed in this incident, to identify the potentially affected individuals, to put in place resources to assist potentially affected individuals, and to provide them with notice of this incident. Authentic Recovery is also taking steps to prevent a similar event from occurring in the future. This includes implementing additional safeguards to further secure all email account information and providing additional training about the proper way to secure information systems.
